This is a simple template for for hotels and travel companies websites. The template includes input for personal information such as name,email, country of origin and the date of arrival and departure. The arrival date uses datepicker with minDate set to the current date of selection -today and the departure date set to a date after the selected arrival date.
A summary of the selection is diplayed besides the form after the selection. You can set this to be sent to email or server.
