I'm one of the co-authors of SemanticDiff, and since you already have difftastic in your list, it might be worth mentioning this tool as well.
It supports fewer languages than difftastic, but goes a bit further than what difftastic can do (detects moved code, hides semantically equivalent code, ...) and is available for VS Code and GitHub.
Not open source, unfortunately, but with a free version available for both target platforms. Let me know what you think :)
