Support Visual Studio 2012-2017

• VSIX is now an optional tool for previewing and generating transforms
• NuGet package performs the on-build transform.
  • Now uses the NuGet /build/ directory for adding SlowCheetah.targets to the project

This release requires manual uninstall of old SlowCheetah. See update.md for instructions.
