This PR is related to this feature request #211343 .
The extensions menu lists any command of enabled extensions.
It is created after vscode loading and it is reactive to these extension events ( install, uninstall, enable or disable ).
If you have advice ( code structure, vscode best practices, ...), comments or suggestions don't hesitate to send it to me.
