Cleanup code in nvmon_perfworks

This is part of making nvmon_perfworks CUDA12 compatible.
Unfortunately the code was fairly unreadable, so this commit fixes some
of the severe code formatting problems. In addition, surplus code is
removed and variables are moved to the lines where they actually are used.
