As noted in #535,
GetNameandGetOrdinalare not currently implemented, leading to the error described in #536 during SQL Bulk Insert of stream reader.This PR includes example files and a modification to ExcelDataReader.cs that implements
GetNameandGetOrdinal.I'm not certain that the current method for determining and saving field names is ideal; feedback and suggestions welcome.
