Splitting a way can still lead to duplicate geometries in OSM database

Some month ago, we had some issues with duplicated geometries created by the parking lanes editor, but I thought that was fixed. But yesterday, I created a duplicated geometry by accident when splitting a road segment – see the street segment "Bayernring" at the eastern edge of this changeset: https://osmcha.org/changesets/133445472/

Unfortunately, I have no idea what I made different than at other street segments I splitted with this editor... So I can't say what can lead to this phenomenon. Maybe a reload of already changed data or similar..?
