404.md only renders at playbook.cio.gov/404/

Problem

The 404 page designed for this project (404.md/404.html) only appears when going directly to https://playbook.cio.gov/404. GitHub's 404 page contents appear in all other cases where a page that doesn't exist at playbook.cio.gov.

Impact

Github's 404 page could prove confusing to someone trying to access the Playbook as there are no links to go to any site except Github.
