Did you have nitrogen turned on in alien bases? If so, I think the really quick upwards movement will give you the bends and kill you

Had it been the bends the cause of death would reflect this. I went over the elevator safeguards again just to be sure and found no issues.

Since it is death by unknown creature it is possible that some creature clipped into the elevator from outside and killed you from there. If it happens again, please post your LogOutput.log from your BepInEx folder.
