I'm running macOS 13.0 (Ventura) and I cannot seem to get SaneSideButtons to work. I have a Logitech G903 mouse, if that makes a difference.
I've tried the latest release via the brew cask. I've also tried manually installing 1.2.0-beta.1.
I've given all of the permissions via System Settings and I see the icon in the menu bar on top, but neither the forward nor the backward buttons seem to do anything to Chrome/Finder/etc.
Any idea what I can do to troubleshoot?

I've had the same issues with my Logitech G502 mouse and macOS Ventura. It wouldn't work with the brew installed version, and the beta didn't seem to work either.
My solutions was to follow the steps below to make it work:
Remove the app from the Applications directory
Remove any leftovers in the macOS Privacy & Security settings regarding Input Monitoring and Accessebility. The settings seems to be bugged with me. Without uninstalling the application, I could not unset or remove any of these settings. That's what made me uninstall the application.
