There is currently a bug report template in the folder .github/ISSUE_TEMPLATE/ on the master branch, but the repository lacks an equivalent template for feature requests. A default feature request template could be added through the repository settings by the repository owner (@sampsapursiainen), as instructed in the GitHub documentation (link). Alternatively, a file .github/ISSUE_TEMPLATE/feature_request.md with the wanted structure could be pushed to the default master branch to achieve a similar effect.
