Unzip into your PDI folder.
The plugin will be available in plugins/Neo4JOutput, samples in samples/transformations/Neo4J.
The sample jobs loads data from the SampleData database to your local Neo4J database. Change Neo4J credentials and you should be good to go.
Tested with PDI 6.x and Neo4J 2.3 and 3.0
