Update IndexEntity tables used in sorting to include index_name and index_value in their resourceUuid index

Update sort index tables as described here to add column index_name to the resourceUuid column index since it's used as part of join to the sorting tables here, to improve performance on queries that involve sorts.
index_value column could also be added to the index to make it a covering index when used here
