Modify the registration form to hide covid related fields

Describe the bug

Covid related data are not relevant anymore in the patient registration form field's, as per the communication from @aparnacoronasafe we will be using the ICD-11 to identity COVID-19- positive patients in the platform.

To Reproduce
Steps to reproduce the behavior:

1. Go to '...'
2. Click on '....'
3. Scroll down to '....'
4. See error

Expected behavior

• only retain the ticked field in the patient registration form, hide the rest of the input fields
• remove it in the shifting details and sample forms as well ( Disease status ) confirm with @aparnacoronasafe once
• Remove the covid related fields in the backend #7868
