Initial Form does not have a user prompt

After running runFirst, I am able to open the newly created form, but it does not have the user selection field. Since I am running this in the post season, it created the form for week 4.
I then created a form for week 1, which also did not have the user selection field.
Finally, I recreated the week 1 form and it now has the user selection field.
(Tested this on two brand new, unrelated sheets)

Thanks for reaching out. I did just make some adjustments to the script after testing it for the Super Bowl.

The postseason patch hadn't been tested much, and especially not without an existing sheet and picks. The post-season, which begins after 18 weeks of the regular season, is denoted as week 19 (wildcard), 20 (divisional), 21 (divisional championship), and 22 (The Superbowl). Therefore, when you make the form for the upcoming final week of the NFL season, week 5 of the postseason by their API, it needs to be inputted as week 22 in the script to generate a form that will prompt to pick between the Chiefs and the 49ers this season.
