Show warning for blocked requests on privacy browsers

It appears this plugin does not work on browsers that aggressively block ads and certain network requests. It will cause a timeout.

Not sure of the correct solution here but it's been suggested to simply embed the form. That's worth looking into but it may take a rewrite of the plugin.

In the meantime, it's probably better to show a warning, as suggested here.
