Permits not appearing to attach

Hello! When I try to attach permits to an accession record, sometimes it works just fine and other times I search for the permit I want to add, hit the add button, and everything looks fine except I can't actually see the permit showing up in the accession record. If I save it, then it appears, but I never know if it'll be there or not when I save. Today, I did it and it happened again so I tried attaching again and I got some kind of error message when I hit the add button but then when I selected save with no permits (because I couldn't save without selecting this option) then it saved and the permit appeared.

If you can post the error message here then it can be worked on. I personally havent had any issues attaching media or permits to an accession so hard to evaluate. thanks
